Annual Paperwork (MSHSAA Pre-Participation Documentation -- Annual Requirements for All Sports and Activities)

The Student Athletic Handbook is furnished to Chaffee Jr./Sr. High School student athletes so that they may be better informed in matters dealing with eligibility, rules, regulations, and expectations.

The Physical Packet includes the MSHSAA Pre-Participation Documentation -- Physical Examination Form, History Form, Parent Permission and Insurance Forms, Concussion Materials, and Student Agreement (MSHSAA), Concussion Signature, and Emergency Contact Pages.

The Annual Paperwork (MSHSAA Pre-Participation Documentation -- Annual Requirements for All Sports and Activities) must be completed yearly for all Sports and Activities.
